For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC(Center for Disease Control) standards. If you would like to make an appointment, please contact our office by phone 321-454-4440 or email. Our office is currently accepting new patients and emergencies by appointment only.
Financial
All fees are due the same day as the visit. Insurance protocol: If you have Dental Insurance, we will need a copy of your information and the insurance form will be printed after any treatment is paid for and completed. Only your signature is required on the form and then you will mail your form to your insurance company and the reimbursement check will be sent directly to you.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee will be charged if a patient does not show up for an appointment without sufficient notice.
